The Vision Behind Happy Village @ Mei Ling

Happy Village @ Mei Ling is part of a broader effort by NUHS to reimagine how preventive health and community care are delivered. Below, we share more about the vision, partnerships and programmes that make this initiative possible.

Preventive Health and Care Delivery
NUHS, in collaboration with researchers from the National University of Singapore, is bringing healthcare closer to home by exploring ways to empower people to take charge of their health.

We adopt a comprehensive approach to preventive health and care delivery, focusing on health education, medical screenings, vaccinations, and behavioural nudges. This strategy aims to detect health issues early and promote healthy habits to improve the long-term health of residents, which will help residents lead healthier, more active lives.

We believe that building meaningful connections with residents is essential to creating a sustainable and effective system that supports lifelong well-being.
Blended Care Team: Integrating Health and Social Support in the Community

Our Blended Care Team is an innovative, first-of-its-kind care model that combines the expertise of healthcare professionals, social care partners, and volunteers to support residents’ health and well-being holistically within the community.

Since 2024, our Blended Care Team, made up of community nurses, community health managers, physiotherapists, and social care partners, has been working alongside volunteers to support residents in their everyday health and well-being:

  • Engage residents regularly to build trust and encourage healthy habits like staying active and eating well.
  • Proactively reach out to those who may need additional health and social support, including people who have not joined preventive health programmes or find it hard to manage their healthcare appointments.
  • Bridge care gaps by providing personalised care near individuals’ homes, and ensuring that they are well-supported by the care team and their personal support networks.
By being in the midst of the community and reaching out to residents, NUHS hopes to better understand what is keeping them from age-appropriate screening, vaccinations; and for older people, helping them with exercise, good nutrition, and preventing loneliness. This will complement and strengthen the healthcare system and help keep our communities cohesive and robust, especially as our population lives longer.
Professor John Eu-Li Wong, Co-Chair, Steering Committee for the Health District @ Queenstown

Looking ahead, NUHS plans to expand this community-based care model, working with partners across the western region of Singapore to support over 1.1 million residents by bringing care and support closer to home.

Community Health Programmes and Activities

We believe that health is more than treating illness – it is about enabling every person to lead a meaningful and active life in their own community. With that belief, our range of health programmes and social activities offered at Happy Village @ Mei Ling encourages residents to stay active, and well. These include:

  • Health Consultations – Make appointments with our Blended Care Team for health counselling, education, and chronic disease management.
  • Circuit Exercises – Led by NUHS geriatricians and physiotherapists, these sessions adapt the VIVIFRAIL© programme, an internationally recognised exercise plan to improve mobility and reduce fall risks. These exercises use everyday household items, making it easy for anyone to continue these exercises at home.
  • Weekly Walking Group – Organised with SportSG and co-led by residents, the programme incorporates light strength and balance exercises alongside the walks.
  • Centre-Based Exercises – Weekly sessions focusing on improving mobility, flexibility, and strength.
  • Health Talks and Workshops – Monthly sessions featuring partners like SportSG and allied health professionals, covering essential topics such as nutrition, disease prevention, and maintaining an active lifestyle.

Guided by the Ibasho philosophy, which promotes socially connected and age-inclusive communities, Happy Village @ Mei Ling empowers residents to not only participate in existing activities, but also to contribute by initiating and leading their own interest groups and activities.
