At NUHS, caring for the community means more than treating an illness. It is about supporting 1.14 million residents in the western region to live healthier, longer, and better lives. Through close partnerships with polyclinics, general practitioners, community care providers, and social agencies, we bring health into the heart of our neighbourhoods.

Population health recognises that community wellbeing is shaped by more than medical care. Social circumstances, the environment we live in, and access to support all influence health outcomes. By addressing these wider determinants of health, we can reduce disparities, prevent illness, and ensure fairness in care.

Our approach is anchored in the NUHS Care Plan, which focuses on keeping our population healthy through education and preventive care, strengthening care in the community by reducing avoidable hospital visits, and improving patient experience through better coordination across providers. This whole-of-NUHS and whole-of-community effort enables us to work with residents, partners, and providers to support healthier living at every stage of life.

Whether through prevention, care transitions, or collaboration with community partners, our aim is to empower residents to take charge of their health, build resilience, and shape a stronger, healthier tomorrow together.
