As part of the Health Together initiative, NUHS works in close collaboration with community partners, healthcare providers, and volunteers to deliver accessible care and support to the heartlands. Through our community care programmes, we support residents in Singapore’s western region by offering timely follow-ups, health screenings, wellness activities, and chronic condition management.

Together, we aim to empower 1.14 million residents to take charge of their health and live well, with the right resources, support, and care—closer to home.
