Due to the evolving COVID-19 situation, we are implementing extra precaution for the safety of our patients and visitors. In line with the heightened COVID-19 measures, the
visitor policy and travel restrictions have been updated.
Clinic Patients
- One accompanying caregiver is allowed per patient.
All outpatients and accompanying caregivers are required to:
- WEAR THEIR OWN MASKS AT ALL TIMES IN ALL PATIENT CARE AREAS. Those without masks will be denied entry.
- Practise hand hygiene.
- Maintain social distancing of at least 1 metre from other accompanying caregivers and patients.

Before entering the patient care areas, all visitors/caregivers/patients will undergo temperature screening and complete a declaration form* on your current health status, recent travel and recent contact with COVID-19 patients.
*Online Declaration Form Patients and accompanying caregivers with Specialist Outpatient Clinics (SOC) appointments must fill up the health declaration form online on the day of the visit to Jurong Medical Centre. Please read the instructions below on how to do so:
- The online declaration form has to be completed by the patient and accompanying caregiver
on the day of the outpatient appointment.
- On the online declaration form, please select “Jurong Medical Centre” and complete all fields. Click on/tap 'Submit'.
- After submitting the online declaration form, a message will appear on your browser. Please keep the browser open as you will be required to show the ‘results' page on the browser at the temperature screening stations before entry. Alternatively, you may take a screenshot of the message.
- If you did not manage to capture a screenshot of the message or if your online submission was unsuccessful, you will be required to complete the hardcopy declaration form onsite. During this period of time, you may experience a longer waiting time at the temperature screening counters.

Under the Infectious Disease Act, it is an offence to provide false information in the health declaration form. Effective 25 June 2021, visitors/caregivers who - are unwell (fever, cough, sore throat, runny/blocked nose or breathless)
- have had close contact with a COVID-19 patient in the last 14 days
- are 12 years and below
- have been on a 'cruise to nowhere', or travelled outside of and returned to Singapore within the last 14 days
- have had exposure to premises where large active COVID-19 clusters^ were located, received an exposure SMS alert from MOH and do not have a valid negative swab test result done after exposure duration
will not be allowed to enter patient care areas.
