What is Social-Health Shared Care Connector? 

An integrated approach where NUHS Community Care Team, social care partners (e.g. Active Ageing Centre), and trained individuals from the community form one coordinated support system to help residents manage their chronic conditions and stay well in the community.

What makes us different?  

Integrated health and social support 

We move beyond addressing health and social needs in silos. The Social-Health Shared Care Connector brings both together, recognising that residents’ health and well-being depend on meeting their needs as a whole. By working collaboratively, we create personalised care plans, share ownership, and deliver holistic support. 

Care that continues beyond doctor visits  

Care doesn’t end at the clinic. Residents receive regular check-ins, companionship and community-based support, helping them stay healthier and more connected to the community.    

Residents take an active role

We don’t just provide care; we partner with residents to set goals and co-create personalised plans that strengthen their confidence in managing chronic conditions and empower them to take an active role in their health.  

Who we support

Designed for individuals who may need extra help in managing their health, such as those who: 

  • Have poorly controlled chronic conditions (e.g. diabetes, high blood pressure and high cholesterol)
  • Live alone or have no caregiver 
  • Are at risk of social isolation
  • Are at risk of health deteriorations
