Community Health Posts (CHPs) are touch points within your neighbourhood where you can get easy access to health services near your home.
At the CHPs, the community care team from NUHS provides residents with health counselling and education. If you have chronic diseases or conditions such as diabetes, high blood pressure and high cholesterol, we can help you in making better lifestyle choices so you can live better and stay healthy in the community.
Our services include:
- Medication education and management - e.g. packing medication
- Health checks and education - learn how to manage your chronic conditions
- Elderly health and home safety checks
- Caregiver training
- Health monitoring for blood pressure and blood glucose
