Diabetes 1-2-3 is a structured self-management programme led by dietitians from Alexandra Hospital, designed to equip your patients with the knowledge, skills, and confidence to take control of their condition.

The programme focuses on three core pillars of effective diabetes management:

  1. Diet and Lifestyle Management: Practical guidance to improve blood glucose control through healthy eating and physical activity.
  2. Self-monitoring: Helping patients track blood sugar levels and identify early changes.
  3. Medication Adherence: Reinforcing the importance of following prescribed treatments for long term control.

What can your patients expect?

Who is eligible?

  • Singapore citizens/ permanent residents
  • Aged 18 to 75
  • Diagnosed with diabetes
  • Patients are encouraged to attend with one family member/caregiver
  • Patients with no chronic kidney disease

Curriculum

 Workshop 1

Chapter 1: Understanding Diabetes

  • Principles of diabetes management (carbohydrate education, hypoglycaemia management)
  • Importance of self-monitoring
  • Complications of uncontrolled diabetes
  • Diabetes myths

Chapter 2: What is Hyperlipidaemia and Hypertension?

  • Food sources of cholesterol and fats
  • How to reduce salt consumption
  • Importance of fiber

*Includes one education tea break.

 Workshop 2 Chapter 3: Diet Management for Diabetes 
  • Energy balance concept
  • Making healthy choices (eating out and food preparation)
  • Art of portion control
  • Understanding food labels 

Chapter 4: Putting it all together

  • Getting ready for change
  • Goal setting
  • Redesigning your lifestyle
  • Tools for self-management
