Community Care Team - Hospital to Home

NUHS Community Care is a multi-disciplinary team of community nurses, health coaches, family physicians and Allied Health professionals. We help patients transition smoothly from our hospitals to the community with provision of transitional care such as Home Visits. 

It has benefitted more than 15,000 enrolled patients, ensuring that they receive appropriate care in the comfort of their homes.

Where necessary, NUHS Community Care may also refer patients to suitable care providers to ensure continuity of care.

Home Visits

Home Visits are for residents who need support after leaving the hospital or who have ongoing health conditions.

We can help you to:

  • Recover after discharge: Adjust to routines at home, understand your medications and manage your appointments. 
  • Manage chronic conditions: Create a personalised care plan, receive guidance on self-care techniques and lifestyle advice to stay healthy.
  • Stay safe at home: Make your home safer with fall prevention checks and simple strength exercises.
  • Caregiver support: Learn caregiving skills and find tips for managing stress.
  • Comfort care: Receive comfort and guidance for serious long-term illness. 

What Happens After You Sign Up

Once we receive your referral or appointment request, NUHS Community Care will:

  1. Call you to understand your health needs and explain the services available, including any potential costs. 
  2. Arrange your first appointment at home, at a Community Health Post, or through a tele-consultation.
  3. Work with you to create a personalised care plan that supports your health and daily life. 

From the first call to every step along the way, we are here to support you so you can recover well, stay healthy, and remain engaged in your community. 

To find out more about Home Visits, please speak with your doctor or nurse at any NUHS hospitals to obtain a referral.

Our services are for residents aged 18 years and older. Fees may apply for home visits.
