Request for Medical Report

To request for a medical report, a patient’s written consent is required for the release of medical information. Consent has to be given via the application form to request for the medical report, except if the patient is:

If you're writing to us on behalf of another person, kindly complete Form A with one of the following forms (where applicable):

 - The Patient / Child below 21 years old / Spouse / Parent
Form A (PDF, 225KB)
 - Authorisation for Collection of Medical Report
Form B (PDF, 446KB)
- Deceased Patient
Form C (PDF, 513 KB)
 - Patient with Mental Incapacity
Form D (PDF, 781KB)
 - Foreign Worker who has left Singapore or is missing
Form E (PDF, 511KB)


A request can be made via the following methods:
  • Apply online via the OneNUHS Mobile App (under "Medical Report Request" module).

  • Via post to the Medical Records Office, Ng Teng Fong General Hospital, 1 Jurong East Street 21, Singapore 609606 
  • In person at the Medical Report Counter, Ng Teng Fong General Hospital, Tower B - NTFGH Wards, Level 2, Admissions Office 
Alternatively, you may enquire at the Medical Records Office or approach the Medical Report Counter at Tower B - NTFGH Wards, Level 2, Admissions Office for more information.

  • Mon - Fri: 9am - 5pm
  • Sat: 9am - 12pm
  • Closed on Sun & Public Holidays

Please be informed that any request for medical report will only be processed upon the receipt of the:

  • Completed application form
  • Required supporting documents
  • Payment of the processing fees

Collection of Medical Report
If you are collecting the medical report personally at the Medical Records Office, please bring along the following documents for authentication:

  • NRIC of patient
  • If the patient is unable to collect the documents in person, please complete the Authorisation for Collection of Medical Report Form and bring it together with both Identification documents of the patient and authorised representative
Authorisation of Representative to Collect Medical Report
If you wish to authorise a representative to collect the medical report on your behalf, please download and complete the Authorisation for Collection of Medical Report Form. Please present the completed form with the required documents as indicated in the authorisation form upon collection of the medical record.
