Family Medicine Clinics (FMCs) are one-stop medical clinics in the community with enhanced facilities and capabilities. Patients with multiple chronic conditions can be seamlessly transferred from Specialist Outpatient Clinics (SOCs) to FMCs for follow-up care closer to home.
FMCs were set up as a joint public-private partnership between private medical providers and NUHS to provide a one-stop and affordable healthcare services for you. The FMCs have a comprehensive medical team of doctors, nurses and allied health professionals who can help you with your common ailments such as fever, cough and flu, and chronic conditions such as diabetes and hypertension.
FMCs also manage the care of patients who are referred from NUHS hospitals.
Frontier Family Medicine Clinic (FMC)
Set up in 2013 with the Frontier Healthcare Group, Frontier FMC is the first FMC in Singapore. Based on a ‘specialist-minus’ model, Frontier FMC adopts a patient-centred medical home approach, which is primary physician led and team-based. While the primary physician manages the needs of patients holistically, he/she is supported by a multidisciplinary team of healthcare professionals. This team-based care is also facilitated through shared medical records and the use of tele-consultations.

Keat Hong Family Medicine Clinic (FMC)
Managed by Trilink Healthcare, Keat Hong FMC was set up in 2017 and is a one-stop medical facility that provides affordable, holistic and quality primary care in the community. It offers treatment options for both acute and chronic conditions. Some of its services include chronic disease management, minor treatment procedures, diabetic retinal photography and diabetic foot screening. KHFMC also provides specialised services in the management of eye conditions and a mood & anxiety clinic. KHFMC is the first FMC to treat patients with non-complex eye conditions such as early cataract and stable glaucoma, in partnership with the NUH Ophthalmology Department.

Managed by Tzu Chi Foundation (Singapore) in collaboration with JurongHealth Campus since 2016, Lakeside FMC is a one-stop neighbourhood healthcare destination for patients offering services from common ailments to chronic diseases. LFMC provides team-based care by bringing together family physician partners, specialists, nurses, care coordinators, allied health professionals and other related services.
