A/Prof Tan has a broad scope of experience that spans acute hospital care, community based hospital care and primary care. His passion lies in the integration of care across the diverse settings that patients will traverse in the course of their care.

• Adjunct Associate Professor, National University Health System (NUHS)

• Adjunct Associate Professor, Dukes-NUS GMS

• Visiting Consultant, Department of Family Medicine and continuing care, Singapore General Hospital

• Visiting consultant , National University Hospital

• Censor In Chief, College of Family Physicians, Singapore

• Examiner,- Graduate Diploma of Family Medicine, and Graduate Diploma of Geriatric Medicine, -

Masters of Medicine (Family Medicine), National University of Singapore.

• Member- National dementia network; MOH

• Member – Medical advisory committee , Quality Steering committee, AIC

 

 

Post graduate Training

 

• Completed Masters of Medicine (Family Medicine) in 1999 and became a Fellow of the College in Family Medicine in 2003. Spent one and a half year in clinical geriatric training in Australia culminating in a second Masters of Medicine (Geriatrics) UNSW in 2003.

 

Awards

 

• Gold Medal for Masters of Medicine (Fam Med), NUS 1999

• Overseas Training Scholarship - 1.5 years community geriatrics training in Melbourne and Sydney

• Healthcare Humanity Award 2006 by the Courage Fund

• Rajakumar Award for Best Research paper -2nd Asia Pacific Primary Care Research Conference 2010
