The Challenges
As a case manager, you will be involved in case management in the hospital as per established criteria. You will be responsible in providing comprehensive care co-ordination for patients. You will also be responsible for providing patient and family education, daily patient care co-ordination with other members of the health care team, assessment of patients' level of care requirements, and communication of patients' needs to the other providers.
You will work with a team of healthcare professionals to develop, implement and evaluate clinical pathways for a defined set of patients. You will also participate in disease management and utilization review.
The Requirements
- A recognized qualification in Nursing, Medicine or Allied Healthcare disciplines.
- 4-5 years of clinical experience in nursing or related field in acute care setting.
- Prior experience in developing hospital-based clinical pathways and/or care coordination is a distinct advantage.
- Possess word-processing, spreadsheet and basic statistical skills.
- Excellent communication skills and ability to work closely with multi-disciplinary team of professionals are vital to the success of the position.
